Instructions for Filling Out the Forms
Form Completion Guidelines:
Please note that forms that are not completed as required or are missing appropriate supporting documents will not be scored.
The criteria assessed pertain to the candidate and their family’s circumstances during high school (up to age 18 or the candidate’s graduation from 12th grade).
The online registration form, available at the end of this section, must be completed in Hebrew only.
The application requires a non-refundable fee of 180 ILS. Please have a credit card ready for payment.
Ensure that a valid email address is provided in the form, as notifications for completing missing materials and the final scoring letter will be sent via email.
A personal explanation letter can be included in the designated section during the registration process.
Required Documents to Attach:
During the completion of the registration form, candidates must upload various identification and supporting documents related to personal and family circumstances. It is recommended to prepare the files in advance before starting the form.
Mandatory Documents:
- Candidate’s ID card, including the appendix.
- Parents’ ID cards, including the appendix indicating the number of children in the family. If the appendix is unavailable, an official certification of the number of children can be obtained from the Ministry of the Interior.
- High school diploma or a letter from the school confirming the years the candidate studied there. Graduates of yeshivas must provide a letter confirming their studies at the yeshiva until age 18.
- Residence address during high school: Provide a certificate from the Ministry of the Interior confirming the place of residence. If there is no change in residence from high school to the present and the address listed on the ID is the high school residence, no additional confirmation is needed.
- Proof of payment.
Documents Related to Special Circumstances of the Candidate or Immediate Family Members (Parents, Siblings):
- Illnesses: Provide an official medical document detailing prolonged/chronic/severe illnesses, including a diagnosis and illness duration (from the treating physician, hospital discharge summary, or other relevant authority). If the illness qualifies for a disability percentage, include the relevant certification.
- Disabilities: Provide certification from the National Insurance Institute (Bituach Leumi) indicating the percentage and duration of the disability. A disability certificate alone is insufficient.
- Divorce: Submit a certificate from the Ministry of the Interior, a divorce certificate from the Rabbinical Court, or a court-issued divorce decree. If these are unavailable, provide proof of single-parent benefits received during the candidate’s high school years.
- Orphanhood: Provide death certificates of the parent(s).
- Sibling’s death: Provide the sibling’s death certificate.
- New Immigrant: Provide the candidate’s or one parent’s immigration certificate, which includes the candidate’s details. Graduates of Na’aleh programs must include proof of immigration under the Na’aleh framework.
- Other Family Situations: Submit a report from a social worker, welfare agency, school counselor, or another official authority describing the candidate’s and family’s situation during the relevant period. For any other unique personal or family circumstances, include the appropriate documentation in the designated section of the registration form.
Upon submission of the form, an acknowledgment will be sent to the provided email address. At the end of the process, the candidate will receive the scoring letter via the email address provided in the form.
Submission Outcome:
The results will automatically be forwarded to the universities (Hebrew University, University of Haifa, Technion, Ben-Gurion University, and Tel Aviv University) through the Association for Educational Advancement.
